DISCLAIMERS & POLICIES

Autonomous APRN Practice Disclosure

Cheval Wellness is an independent clinic led by Angela Mineo, DNP, APRN (or other licensed APRNs). We provide primary care as an autonomous Advanced Practice Registered Nurse under Florida law (Florida Statute § 464.0123).

This means:

  • No on-site physician supervision or supervisory protocol is required.
  • Our APRN holds an active Florida APRN license, national board certification in Adult-Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner, and has completed all requirements for autonomous practice, including at least 3,000 supervised clinical hours, graduate coursework in differential diagnosis and pharmacology, and no recent disciplinary actions.

This practice does not include physician services. If your condition needs physician-level care, we will refer you appropriately.

This disclosure is provided as required by Florida law for autonomous APRN practices. Questions about our provider’s qualifications? Contact us at 813-776-5751 or info@chevalwellness.com.
